The swallow is a complex neuromuscular event. A symphony of nerve impulses travel back and forth between the cortex, the brainstem and the muscles in order to prepare the bolus and then guide it safely down towards the esophagus. Clinicians who treat swallowing disorders will seek to engage this system for therapeutic purposes; facilitate a muscle group, inhibit another; and enhance the feedback to retrain certain movement patterns, etc. The skillful application of such therapeutic interventions requires a thorough understanding of the anatomy involved in the swallow movements.
This course is designed to provide dysphagia professionals with the background necessary to participate in advanced courses that require detailed knowledge of swallowing anatomy. This information, will build the therapist's knowledge and skills for treatment of swallowing disorders and will provide the course participant with a stronger basis for a comprehensive examination of the dysphagic patient. The information provided in the course will also facilitate professional communication between the swallowing professional and other health care professionals involved in the treatment of dysphagia.

Current research will be reviewed to explore how the NMES parameters such as frequency, phase duration and duty cycle can be changed to maximize impact. Participants will learn about the benefit of using up to 4 channels of stimulation and will learn how to sequence the stimulation between channels to mimic a normal swallowing sequence. In addition, two new modalities will be reviewed and practiced: sEMG Biofeedback and sEMG triggered electrical stimulation.

On average, one of out every 700 infants is born with a cleft lip and/or palate. Speech therapists are involved in a patient's care from the beginning since babies with cleft lip and/or palate have feeding difficulties and often require the use of a specialized feeding system. The four D's - dentition, dental care, dysphagia and diet are intertwined. This seminar will build on our basic knowledge and delve deeper into the role of dentition in maintaining our patient's health. What role does each tooth play in mastication? What disease processes wreak havoc on our patient's dental health? Dental care or oral hygiene takes a toll on our patient's health as well. Not only is dental care imperative for all individuals on our case load, but it is especially critical for individuals with specific diseases. We will discuss the impact of poor dental care on overall health. Individuals on our dysphagia caseloads have any combination of natural dentition, partial or full dentures or are edentulous. What role does dentition status and dysphagia play in diet selection? How does lack of dentition increase the patient's risk of aspiration? Rather than continuing to use the guidelines for the National Dysphagia Diet, the IDDSI should be implemented in our facilities. We will discuss how these two diets compare, and how to educate our staff to recognize and/or test foods to ensure the diet and liquid standards are met. New products that match the IDDSI levels will be introduced and reviewed. Many times, the patient and/or family reject the diet modification recommendations. Some facilities offer waivers, which are not recognized by the courts. Better options we implement are the Shared Decision-Making Algorithm and The Care Planning for Resident Choice.

This interactive course will take the fear out of helping adult patients with severe dysphagia who require tube feeding. Participants will leave this course with a strong foundation, and a better understanding, of the many patient and family considerations involved with tube feeding. The instructor will use lecture, videos, pictures and case studies. Information for clinicians such as SLPs, OTs, Dysphagia Specialists and Nurses is provided to help them support patients and families who may feel overwhelmed facing, or living with, a feeding tube. Course material will include topics such as history, tube basics, feeding methods, tips and techniques, common complications, equipment care, medical considerations, the heart and mind connection during mealtimes, the role of water, the gut and brain connection, diets, traveling with a tube, the role of the dysphagia therapist and treatment considerations. The psychology of tube feeding, impact on the family, and mealtime dynamics will also be discussed. Participants will leave with a new confidence in helping patients, families and team members through the world of tube feeding.

Literature over the last 2 decades has shown an obvious link between motor function and cognition. Emerging data suggest therapies designed to improve specific aspects of cognitive function, may also improve gait, increase use of compensatory strategies, and reduce fall risk. Falls and fall-related injuries are a huge concern and costly burden to individuals, society, and healthcare systems. Despite the costly and physical consequences of falls, common interventions and 'fall prevention programs' have been shown to be inadequate or ineffective. This evidence shines a new light on the SLPs ability to target functional safety goals through cognitive therapy to change the outcomes of patient safety. The evidence-based techniques reviewed in this course are applicable to patients with fall concerns who mobilize in any way and/or participate in ADLs, including patients with Dementia, Parkinson's Disease, stroke, traumatic brain injuries, and other neurological diagnoses. This course focuses heavily on evidence-based cognitive therapy techniques to improve patient safety; along with education essentials for training of staff/family as a part of your plan of care. Discussion will include how to target these skills without duplicating services with PT/OT. Course material will cover goal writing for best practice standards and avoiding insurance denials. Pediatric Feeding Disorder (PFD) is a new diagnosis that not only encompasses dysphagia, but also dysfunction in medical, nutritional, psychological, and/or feeding skills domains. Feeding therapy providers are seeing a rise in this population. Outcomes are more successful with the right skills to assess and treat infants with feeding challenges. This course will discuss the comorbidities of PFD, caloric fortification, observations of distress, positioning, and the four domains associated with this diagnosis (nutritional, psychological, medical, and feeding). Neuromuscular Electrical Stimulation (NMES) is a relatively recent addition to the treatment toolkit of dysphagia clinicians. The modality was first introduced in a significant way in 2003 and has since seen a rapid adoption among Speech and Language Pathologists. It is estimated that over half of clinicians working with dysphagia have attended a continuing education course in the use of this modality.
Because of this growth and ongoing reports of positive patient outcomes in the peer-reviewed literature, the product and educational options have increased. Clinicians can now purchase different devices and they can choose to attend different continuing education courses.
It can be challenging for the clinician to discern what the best option would be. How do the devices compare? What are the therapeutic opportunities offered by each of them? Feeding problems complicate the care of preterm and term infants in the NICU. Delayed oral feedings can lead to prolonged length of stay, need for G-tube, family stress and separation. These infants, typically demonstrate discoordinated suck-swallow-breath patterns, delayed oral motor skills and weak pharyngeal muscle movements - thus leading to penetration and aspiration, putting the infant at increased risk of pneumonia and future oral aversion. Traditional therapies have shown limited and slow progress in this fragile population. Infants unable to demonstrate adequate feeding abilities are often discharged with G-Tube and PEG placements. The use of NMES as part of feeding and swallowing therapy has grown exponentially over the last 20 years across all ages. Use of this modality in the NICU is also becoming more frequent. This short course will focus on the objectives of this therapy and how it can complement traditional therapies in the NICU when these neonates/infants struggle with dysphagia and discuss opportunities to help this fragile population perform their best.

Whether you are currently working with tracheostomy and ventilator dependent patients or if you would like to develop the knowledge and skills for this specialty, this course is for you! The course material will include types of tracheostomies, speaking valves, and treatment strategies to help your patients wean off the vent and successfully decannulate. Therapists working with patients with trachs and vents can be challenged with getting instrumental assessments, choosing treatment strategies, and communicating the role of the therapist for this unique population to multidisciplinary teams. This course will present the physiological and structural changes of the trachea following a tracheostomy. It also offers guidelines and treatment strategies to initiate po intake and achieve voicing using speaking valves. Participants will walk away with new ideas and understandings ready for tomorrow.

Your patient complains of double vision, and dizziness. The chart says your patient is "legally blind". Your patient is having problems reading. These are common issues for adult patients of the rehab therapist, but the root causes of these issues are frequently misunderstood, so interventions are not effective. Therapists are presented the functional problems of visual dysfunction every day. Many therapists do not recognize these complaints as visual issues due to a lack of understanding of the changes that may occur in the adult visual system.
This course will help therapists recognize how naturally occurring changes in the visual system affect function. They will learn the questions to ask when your patient tells you they had cataract surgery, or they see double. Attendees will know the visual issues that can cause falls or reading problems and how to assess and improve the visual system so their patient can continue the activities they love. Attendees will learn to assess the adult visual system and develop activities to improve the most common visual complaints. They will even understand glasses better and be able to help their patient pick the eye care professional to best manage their visual complaints. This course will make you into a vision-aware therapist so you can recognize visual dysfunction better and provide interventions that get results where others have not.

The first is John Michael and Vickie Night from "JM Strong 24" and Shannon Vogt, NMES Dysphagia Specialist
Shannon Vogt, John Michael's Speech Pathologist, presented her clinical strategies which brought John Michael from peg tube dependent to a successful outcome that experts said will never happen. John Michael was a senior in high school and having just signed his National Letter of Intent to play D1 lacrosse, life was good. Then one December morning it all changed. A massive brain stem stroke left John Michael locked-in, unable to speak or move anything but his eyes. With the worst possible prognosis, John Michael has defied the odds, including having the feeding tube removed following the dedication of his skilled dysphagia therapist.
The second presentation is Meredith Pauly and Chef Christopher Kearse discussing a fine dining experience for people with dysphagia and their families and the backstory of the Dinner with PALS (Persons with ALS) event, . Learn how they did it and get inspired to find a place at the table for people with swallowing disabilities in your own community.
